HOW TO APPLY FOR MEMBERSHIP

- Any individual 18 years old & above can fill out an application form at the Club Ultima
   Membership Office
- Present valid ID (Company ID, SSS, Driver’s License, or Club Ultima membership club)
- Prepare payment. For check payments, it should be made payable to
   Club Ultima Philippines, Inc.

TRANSFER OF MEMBERSHIP RIGHTS

Due to the exclusive nature of the Gold or Platinum Membership, Member is not encouraged to sell his membership right. However, Club gives Member an option to sell or donate his membership to his beneficiaries or third person upon prior notice to Club. Transferees shall acquire all the rights and privileges under this contract, and shall be overed by the provisions herein stated. Member shall pay a processing fee of two thousand pesos (P2,000) for the transfer.
